RESOLUTION

To resolve this issue, follow these steps:
  1. Click Start, point to Settings, and then click Control Panel.
  2. Double-click Add/Remove Programs.
  3. In the Add/Remove Programs Properties list, click OnStream DI-30, and then click Add/Remove.
  4. Click Uninstall to remove OnStream DI-30.

NOTE: If the icons are still not displayed as expected, right-click a blank area of the Office Shortcut Bar, and then click Refresh.
