OL98: Outlook 98 Access Violation After Spell Check Button Added to New Note Toolbar (256115)

When you customize the toolbar of the New Note form in Outlook 98, under certain specific circumstances, you can experience an access violation that causes
Outlook to stop responding. The specific circumstance is:
After you customize the toolbar, you return to the main Outlook window, click New Appointment, and immediately click the Attendee Availability
tab of the new Appointment form. An access violation occurs.
RESOLUTIONA supported fix is now available from Microsoft, but it is only intended to correct the problem that is described in this article. WORKAROUND
To avoid this issue, do not put the Spell Check button on the toolbar of the New IPM.Note form.
