	1. The system hangs when booted with the LVM
	   (Logical Volume Manager) maintenance mode
	   option, "boot vmunix -lm".
	2. The following message is displayed on the console:
	   "ia64dsk_strategy: Attempt to access partially open
	   partition".

Defect Description:
	PHKL_28787:

	In the SCSI partition driver, the logic for
	checking invalid partition fails while
	booting in the LVM maintenance mode.

	Resolution:
	The driver code has been modified to detect invalid
	partitions.

	( SR:8606284731 CR:JAGae48676 )
	A field used for performing retries is not initialized
	in the SCSI bus structure by the dump driver.
	Therefore, the crash dump is not saved when
	the system panics.

	Resolution:
	The driver code has been modified to initialize the
	retry count to ensure that retries happen as expected.

	1. Back up your system before installing a patch.

	2. Login as root.

	3. Copy the patch to the /tmp directory.

	4. Move to the /tmp directory and unshar the patch:

		cd /tmp
		sh PHKL_28787

	5. Run swinstall to install the patch:

		swinstall -x autoreboot=true -x patch_match_target=true \
			  -s /tmp/PHKL_28787.depot

	By default swinstall will archive the original software in 
	/var/adm/sw/save/PHKL_28787.  If you do not wish to retain a
	copy of the original software, include the patch_save_files
	option in the swinstall command above:

		-x patch_save_files=false

	WARNING: If patch_save_files is false when a patch is installed,
		 the patch cannot be deinstalled.  Please be careful
		 when using this feature.
