PHKL_15642: The most visible symptom was a panic; in other cases, invalid EIO errors were being returned. Defect Description: PHKL_16209: Three more race conditions have been found in the UP emulation code. One of these, which causes a panic in requeue_to_pool, was encountered when testing a driver currently under development. The other two were found by code inspection. PHKL_15642: There were 3 different race exposures in the UP emulation code where global data was being interrogated then locked and changed then unlocked. However, the lock needed to be held across interrogating and changing the data. The LVM ioblast test reproduces the problem quite nicely.
